I authorize PHA or a representative of PHA to: include my photograph in any of PHA’s printed materials; include my photograph and/or audio/video recording in presentations at PHA related meetings; include my photograph and/or audio/video recording on any websites owned or hosted by the PHA; include my photograph and/or audio/video recording in other electronic formats (such as CD-ROMs or audio/video downloads) created or sponsored by the PHA include my full name and affiliation with any use of my photo or audio/video recording use my name in connection therewith if they so choose; and/or edit, alter, copy, exhibit, publish, or distribute said Media for any lawful purpose and that this authorization extends to all languages, media, formats and markets. I waive any right to inspect or approve any product in which my likeness appears.
